Revisions in Reimbursement - Effective April 5, 2004, for services performed on or after January 1, 2004, the Centers for Medicare & Medicaid Services (CMS) have made revisions in reimbursement to the following CPT codes and CPT codes with a HCPCS modifier on the 2004 Medicare Physician Fee Schedule Database (MPFSDB).
